Passports

When applying for a Passport, the agent requires an appointment. Please call 573-378-4413 or 573-378-6839.

ITEMS REQUIRED WITH APPLICATION
-Proof of U.S. Citizenship: Certified Birth Certificate or previous U.S. Passport
-Proof of Identity: Driver's License
-Recent 2x2 Color Photograph: (maybe taken at Wal-Mart or B & W Graphics)

-Book (cost is $130.00 for adult payable by check or money order)
-Card (cost is $30.00 for adult payable by check or money order)
-Book and Card (cost is $160.00 for adult payable by check or money order)
-Service Fee (cost is $35.00 payable by check, money order or exact cash)
FEES ARE LISTED ON DEPARTMENT OF STATE WEBSITE AT travel.state.gov. BY LAW, THE PASSPORT FEES ARE NON-REFUNDABLE.

REQUIREMENTS FOR CHILDREN
To submit an application for a child under age 16 both parents or the child's legal guardian(s) must appear and present the following:
-Proof of U.S. Citizenship: Certified Birth Certificate
-Proof of Identity on both parents: Driver's License
-Book(cost is $100.00 for minor payable by check or money order)
-Card (cost is $15.00 for minor payable by check or money order)
-Book and Card (cost is $115.00 for minor payable by check or money order)
-Service Fee (cost is $35.00 payable by check, money order or exact cash)

REPORT YOUR LOST OR STOLEN PASSPORT BOOK OR PASSPORT CARD!

PASSPORT VALIDITY LENGTH
If you are 16 years of age or older: Your U.S. passport will be valid for ten (10) years from the date of issue except where limited by the Secretary of State to a shorter period.
If you are under 16 years of age: Your U.S. passport will be valid for five (5) years from the date of issue except where limited by the Secretary of State to a shorter period.
